The Job

San Bernardino County invites qualified professionals to apply for the position of Enhanced Care Management (ECM) Registered Nurse Care Manager (RNCM) for Arrowhead Regional Medical Center, the Department of Behavioral Health, and the Department of Public Health. This position will act as the point of contact primarily for patients with the highest complexity physical health conditions. This dynamic career opportunity offers a unique way to provide patient care and collaborate with physicians and other healthcare professionals to manage patients with mental health, substance use, chronic disease processes and transitions of care within the population health management model. RNCMs assigned to the ECM program support a caseload of patients with complex medical needs by providing clinical oversight, individualized health education, and ongoing care coordination.

Typical duties include, but are not limited to:

  • Serve as a primary point of contact for patients with significant physical health conditions, conducting medication reconciliation, assisting with transitions of care after hospitalizations or emergency department visits, and communicating directly with primary care providers to address concerns and advocate for patient needs.

  • Conduct assessments including the Comprehensive Health Assessment, Patient Health Questionnaire-9, and Brief Addiction Monitoring; develop shared care plans, and track clinical outcomes using electronic health record and managed care platforms.

  • Collaborate closely with Clinicians, Community Health Workers, and other ECM team members, offering clinical guidance, reviewing medication plans with CHWs, and ensuring recommendations are communicated appropriately to providers.

  • Maintain regular engagement with patients through in-person and telephonic contact, including patient and family education on disease processes, treatment options, and chronic disease management.

  • Use motivational interviewing to promote patient participation in their health goals.

  • Prepare and present patient cases during Systematic Case Review meetings.

  • Support warm handoffs to ECM team members, coordinate care with external healthcare providers, and contribute to smooth transitions across care settings.

  • Drive personal and County vehicle throughout San Bernardino County, and at times outside of this area, for patient visits and appointments.

The Enhanced Care Management (ECM) benefit is a foundational component of CalAIM. ECM is a whole-person, interdisciplinary approach to comprehensive care management that addresses the clinical and non-clinical needs of high-cost, high-need managed care members through systematic coordination of services that is community-based, interdisciplinary, high-touch, and person-centered.

The ECM program is a collaboration between the following San Bernardino County departments: Arrowhead Regional Medical Center, Department of Behavioral Health, Department of Public Health, Sheriffs Department, and the Probation Department.

CONDITIONS OF EMPLOYMENT

Pre-Employment Process: Prior to appointment, applicants must pass a background investigation, which includes fingerprinting and medical exam, and may include drug and/or TB testing. Applicants must be a U.S. Citizen or a permanent U.S. resident.

Shifts: Monday - Friday 8:00AM-5:00PM with possible schedule variation as needed for mobile unit or community events.

Travel: Travel throughout the county is required depending on assignment. Employees will be required to make provision for transportation (mileage reimbursement at current IRS rate is available). At the time of hire, a valid California driver's license and proof of automobile liability insurance must be produced and maintained for the individual providing the transportation.

Minimum Requirements

RN License: Must possess and maintain a current license as a Registered Nurse with the State of California Board of Registered Nursing.

BLS Certification: Must possess and maintain a Healthcare Provider level Basic Life Support (BLS) certificate (Course Completion Card) issued by the American Heart Association or the Red Cross.

Experience (Must meet one of the following two options):

Option 1: Three (3) years of registered nurse experience in an acute care hospital.

  • OR-

Option 2: Two (2) years of registered nurse experience in a Primary Care, Outpatient Care, Care Management or Community Health setting.

Desired Qualifications

Candidates possessing a Bachelors or Masters degree are desired.
