We are growing our Enhanced Care Management (ECM) program at Elica! ECM is a key part of CalAIM's statewide Medi-Cal benefit available to select "Populations of Focus" with complex needs and who are facing difficult life and health circumstances. This program is focused on breaking down the traditional walls of health care -
extending beyond hospitals and health care settings into communities. ECM addresses clinical and non-clinical needs of the highest-need enrollees through intensive coordination of health and health-related services and will meet beneficiaries wherever they are - on the street, in a shelter, in their doctor's office, or at home.
WHAT YOU'LL DO
Under the guidance of the CalAIM Manager, the Enhanced Care Management (ECM) Supervisor's overall role is to communicate organizational needs, oversee employees' compliance with the CalAim ECM and Managed Care Plans (MCPs) requirements, provide guidance on compliance with the CalAim ECM and MCP requirements, support compliance with the CalAim ECM and MCP requirements, identify development needs, onboard and train new team members and supervise the ECM team's day to day operations ensuring the successful completion of program requirements.
The ECM Supervisor focuses on supervising and daily operations and staff's compliance in providing care coordination/case management services to clients enrolled in the ECM program. The ultimate goal of all services provided is assisting the client with obtaining medical and behavioral health support with medical and community resources referrals, income, medical insurance, social service supports, and housing referrals. All goals are managed by development and implementation of a client centered care plan. The ECM Supervisor, in collaboration with the ECM Clinical Consultant, is responsible for overseeing the development and implementation of the patient centered care plan by the ECM Care Managers.
The ECM team is a part of the Social Care Department and works in close collaboration with the Community Supports team, other support programs, Elica's departments and services, and community partners.

Compensation Disclosure: $70,720.00 - $85,280.00 Annually
The compensation range listed reflects Elica's good-faith estimate of the anticipated hiring range for this position in accordance with California pay transparency requirements. Final compensation will be determined based on job-related factors, including relevant experience, education, certifications, skills, and qualification.

This is a fully on-site, office-based position (North Highlands, CA 95660).
Scheduled Hours
Oversee staff with shifts scheduled between the hours of 7:30 am to 6:00 pm Monday through Friday

Physical Requirements
The work environment is office, clinic and field based administering program education and Housing Navigational Services to Adults, Children and Youth experiencing homelessness or at risk of homelessness. Work environment includes office, clinic, hospitals/facilities, client homes, streets and homeless encampments, and homeless shelters. Employees are to adhere to field visit policies, including, but not limited to being accompanied by a colleague while working with clients in a not public setting. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to walk; use hands to finger, handle, or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment can range from quiet to moderately loud; the incumbent must be able to focus in an environment with many distractions.
The employee may be required to run agency related errands and attend off-site meetings; the employee must be able to operate, maneuver and/or control a motor vehicle. In performing the driving responsibilities, the driver may sit for long periods. This requires intense concentration, particularly in poor driving conditions.
The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health. The employee may experience a number of unpleasant sensory demands associated with the client's use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bio-hazardous materials (bodily fluids including blood and urine) and hazardous chemicals. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations.
