    Description

    The Emergency Department Clinical Social Worker 2 delivers discharge planning and social work services during normal business hours and the entire hospital after hours. This job requires interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric. This job provides direct intervention with patients and families and jointly with the health care team, provides assessment, problem solving, advocacy, community referrals and crisis intervention services. Responsible for the daily management of all complex discharge planning and social work functions.

    Licensure, Certification, or Registration Requirements for Hire: Licensed Clinical Social Worker in the State of Virginia preferred Licensure, Certification, or Registration Requirements for continued employment: N/A Experience REQUIRED: Minimum of one (1) year of clinical social work experience Experience PREFERRED Clinical social work experience in an acute care setting Related experience in acute discharge planning, home health, case management, long term care or related community agency Education/training REQUIRED: Masters of Social Work Degree from a Council on Social Work Education accredited program Education/training PREFERRED: N/A Independent action(s) required: Primary responsibilities include proactive development, implementation and ongoing revision of an interdisciplinary plan of care for patients who receive care in the Emergency Department. Negotiates and collaborates with the interdisciplinary team, community resources and payers to facilitate timely and cost effective discharge plans that enhance appropriate health and social services in the continuum of care.
