Must be able to work a minimum of 8 shifts/ mo

Provider will work at the main facility as well as affiliated FSED .
Facility Information:
Census: 150/day
Daily Admissions: 35-40
Shift time: 10 & 12 hours, AM and PM shifts available
Privs:
Emergency privs with clean file (approximately 45 days) . S tandard privs approximately 90 days.
Travel/Lodging: Hotel and mileage reimbursement
. Flights and rental car on case-by-case basis.
$2,000 referral bonus received for any referral who works 160 hours.
