Empanelment Analyst

The Empanelment Analyst supports the organization's provider empanelment program by maintaining accurate patient-provider assignments, monitoring panel performance, and developing reports and dashboards to support operational decision-making. This position analyzes panel data, supports continuity of care initiatives, assists with provider capacity management, and collaborates with clinical and operational teams to ensure the accuracy and integrity of empanelment data.

This role is located in Midtown - Sacramento, CA (95811).

This role allows a hybrid schedule requiring 3 days per week on-site.

ESSENTIAL FUNCTIONS

  • Generate monthly panel reports and review PCP assignments
  • Calculate continuity of care percentages for each provider; adjust for vacation coverage schedules and other factors
  • Resolve unassigned patients by reviewing appointment history (and possibly the clinical record) to determine appropriate assignment
  • Monitor influx of new patients into provider panels and analyze provider capacity to absorb new patients; review issues with VP of Health Services and Chief Health Officer and reassign as appropriate.
  • Assess panel size against "right size" and discuss any needed adjustments with the VP of Health Services and the Chief Health Officer and/or the medical management team
  • Track patient status changes including death, transfer to other care, and make needed changes in the practice management system
  • Review transfer of care requests, including:
  • Patient-initiated PCP change requests; facilitate discussion between and obtain approval from current assigned PCP and future PCP; make the change in the practice management system
  • Provider-initiated PCP change requests. facilitate discussion between current PCP and potential future PCP; obtain agreement of acceptance by the new PCP; make the change in the practice management system
  • Track provider status changes (transfer, termination, sabbatical, medical leave, etc.) and:
  • Evaluate impact on assigned panel of patients
  • Reassign panel to other clinic providers according to panel capacity
  • Notify affected patients of new PCP assignment
  • Collaborate with clinic managers, providers and care teams in the implementation and ongoing maintenance of the provider empanelment process
  • Assist in the development of workflows and protocols to maximize outcome reporting
  • Owns the panel reporting process, including data cleanup, provider alignment, and validation to ensure accuracy and integrity of empanelment data
  • Develops dashboards and reporting tools to support panel management, empanelment, and risk stratification, utilizing systems such as Azara, EPIC OCHIN, and BI reporting tools
  • Partners with IT and Population Health teams to optimize data sources and reporting workflows that support patient outreach, recall initiatives, and care gap closure
  • Develop and implement data strategies that optimize efficiency and data quality
  • Assist with daily review appointment to ensure patients are scheduled with PCP
  • Adhere to attendance and punctuality policies
  • Maintain established departmental policies and procedures
  • Support the vision, mission and goals, and demonstration commitment the values of the organization
  • Enhance professional growth and development through participation in educational programs, current literature, in- service meetings and workshops

MINIMUM REQUIREMENTS

  • Bachelor's degree in healthcare, business, public health, analytics, or a related field; or equivalent combination of education and experience.
  • Two years of experience in healthcare operations, healthcare analytics, panel management, population health, or related field.
  • Intermediate advanced proficiency in Microsoft Excel.
  • Experience working with healthcare data, reports, and operational metrics.
  • Strong analytical, organizational, and problem-solving skills.
  • Ability to interpret data and communicate findings to operational and clinical stakeholders.
  • Experience with electronic health records and healthcare reporting systems
