Job Title: Employment Transition Coach
Location: Rochester, NY, 14624
Department: Vocational Services
Reports To: Associate Director of Vocational Services
Employment Type: Full-Time - 40 Hours, 8am - 4:30pm with some evening flexibility for meetings & events
Starting Wage: $20.01-$21.55/hour

Your Core Responsibilities:

  • • Provide daily coaching and group instruction focused on job readiness, workplace behavior, and independence.
    • Facilitate skill-building activities including hand-assembly and contract-based work.
    • Support individuals in developing communication, professionalism, and time management skills.
    • Develop and maintain Staff Action Plans and participate in life plan meetings.
    • Track attendance, productivity, job performance, and progress toward individual goals.
    • Complete all required documentation in compliance with Medicaid, OPWDD, DOL, ACCESS-VR, OSHA, and agency standards.
    • Support transitions to community employment or volunteer opportunities.
    • Collaborate with internal teams to support payroll, quality assurance, and program operations.
    • Maintain a safe, clean, and supportive work environment.
    • Serve as a positive role model promoting inclusion, dignity, and person-centered practices.

What You Bring:

• Associate degree with 1 year of relevant experience OR high school diploma/equivalent with 3 years of pre-vocational or human services experience.
• Strong communication, organizational, and problem-solving skills.
• Ability to work in a dynamic, team-based environment.
• Commitment to supporting individuals with intellectual and developmental disabilities.
• Ability to meet agency driver and background check requirements.
• Ability to stand, walk, and lift up to 40 lbs.
