    Life Insurance - $25,000 coverage

    Medical benefits are effective the first of the month following 30 days of employment

    Retirement Benefits: 401(k) Plan with 4% company match (eligibility begins after 90 days of employment)

    At your employee orientation, you will be provided with more information about the benefits enrollment process, eligibility criteria for these benefits, and the effective dates of coverage. Your enrollment in benefits programs for which you are eligible must be completed within the first 30 days of your employment or you must wait until the next open enrollment period.

    Bonus target is $1,000/quarter. Bonus achievement amount will be determined by a combination of individual performance metrics and overall company performance. Employees are eligible after completing their 90-day probationary period.

    • Note that for all clinical role interviews, we ask if they have any sensitivity to sound or smell. One of the treatment machines presents a loud ticking noise and essential oils are sometimes used.

    8. What is your experience working with patients in the mental health arena?

    9. Why are you successful? What are your biggest strengths?

    10. This position is Monday-Friday in XXX. Do those workdays and your commute work consistently for you?
