Responsibilities

• Conduct patient assessments and assist with ongoing care management

• Administer medications, injections, and prescribed treatments

• Educate patients on disease management, treatment plans, medications, and self-care strategies

• Provide guidance regarding monitoring devices, therapies, and preventive care measures

• Respond to patient inquiries and assist with care coordination activities

• Collaborate with physicians and interdisciplinary healthcare teams to support patient care plans

• Document patient interactions and clinical information accurately and timely

• Promote infection prevention and patient safety standards

• Support patients managing chronic medical conditions through education and follow-up care

• Contribute to a positive and collaborative team environment

Qualifications

• Current Registered Nurse (RN) license or eligibility for licensure

• Graduate of an accredited nursing program

• Current Basic Life Support (BLS) certification

• Previous outpatient or ambulatory nursing experience preferred

• Experience providing patient education preferred

• Strong communication, assessment, and organizational skills

• Ability to work effectively in a team-oriented healthcare environment
