Job Title: Enhanced TASC Care Manager
Location: Fayetteville, NC Cumberland County
Hours: 40 hours/week, Full Time, M-F, Non-Exempt


SUMMARY OF POSITION:

Coastal Horizons Center is seeking a dedicated and qualified Enhanced TASC Care Manager to join our team. The Enhanced TASC Care Manager will perform professional care management duties for justice-involved individuals with primary mental health diagnoses, including individuals with serious mental illness (SMI), participating in the Treatment Accountability for Safer Communities (TASC) Program. This position operates as part of North Carolina’s Enhanced TASC model, implemented in alignment with DMHDDSUS guidance to standardize TASC services for individuals with primary mental health diagnoses and improve service outcomes. The ideal candidate will collaborate with behavioral health providers, justice system partners, hospitals, and community agencies to improve treatment engagement, reduce barriers to care, and support successful outcomes for individuals involved in both the behavioral health and justice systems.


POSITION HIGHLIGHTS:

Primary duties include conducting screenings and assessments, coordinating linkage to behavioral health treatment and community resources, developing person-centered care plans, and providing intensive care management services for justice-involved individuals with mental health and co-occurring needs. This position also includes collaboration with courts, probation/parole, hospitals, behavioral health providers, and other stakeholders to support continuity of care and treatment engagement.


REQUIRED EDUCATIONAL/EXPERIENTIAL QUALIFICATIONS:

  • Education: Four (4) year college/university degree in human services, social work, psychology, counseling, or related field.
  • Experience: Minimum of two (2) years of experience working with individuals with mental health disorders, serious mental illness (SMI), co-occurring disorders, and/or justice-involved populations preferred.
  • Preferred Knowledge and Skills:
    • Knowledge of trauma-informed care, person-centered practices, behavioral health systems, crisis intervention, and community-based resources.
    • Experience collaborating with courts, probation/parole, behavioral health providers, hospitals, and community agencies preferred.
    • Knowledge of recovery-oriented systems of care, harm reduction, and wellness-based concepts preferred.

ADDITIONAL INFORMATION (Required Skills):

  • Strong communication, organizational, and time-management skills.
  • Ability to work independently and manage a caseload effectively.
  • Ability to establish and maintain effective working relationships with diverse populations and multidisciplinary teams.
  • Proficiency in electronic health record systems and remote collaboration tools (i.e., Teams, Zoom, etc.).
  • Ability to accurately document services and maintain compliance with program standards and timelines.
  • Ability to work effectively with individuals experiencing mental health challenges, crisis situations, and justice involvement.
