Responsibilities:

This position oversees the strategic development and operational execution of the PHC Patient Transfer Center which seeks to facilitate the transfer of patients between facilities throughout the Piedmont Healthcare System. This position will set goals and establish key metrics of performance, standardize processes/procedures through policy development and implementation, provide executive guidance and governance of EMR Transfer Center platform, develop a centralized staffing model and scheduling practices, guide evaluation and implementation of additional technologies within Transfer Center process, and develop and maintain strategic partnerships with executive and physician leaders across the health system.

 

What you will do

  • Develop and implement policies and procedures for patient transfers to ensure safety and compliance with all regulatory requirements, industry best practice, and PHC system and hospital policies.
  • Oversee staff and day-to-day operations of the centralized PHC Patient Transfer Center and serve as liaison between PHC facilities for escalation and resolution of concerns.
  • Collaborate with healthcare providers, hospitals, and non-Piedmont facilities and transportation providers to coordinate patient transfers and ensure seamless transitions of care.
  • Monitor transfer center volumes, efficiency metrics, patient leakage, and patient capacity distribution (level loading) for function, improvement, optimization.
  • Oversee transfer center emergency preparedness and transfer protocols aligning with regulatory standards to maintain safe, compliant operations across the system.
  • Ensure entity specific capacity and capability matrices are correctly reflected and updated within the EMR and all centralized communication platforms/technologies on an ongoing basis.
  • Ensure consistency and compliance in transfer center operations by driving all transfer workflow through the EPIC Grand Central EMR platform.
  • Manage budget and financial performance of the center, including transfer volume, reduction of patient leakage, and labor and productivity metrics—including centralized staffing model.
  • Represent the center to internal and external stakeholders, including PHC leaders and facilities, hospitals, healthcare organizations, government agencies, and community partners.
  • Drive operational improvement in patient transfers through optimization of EPIC EMR Grand Central platform and identification/implementation of other support technologies, such as telecom solutions, in the transfer process.
  • Create and manage escalation processes for specialized patient transfers—(Ex. international transfers or VIP patient transfers.)
  • Foster a culture of safety, quality, and ownership at every level to ensure superior patient care and satisfaction throughout the transfer process.
  • Create and manage clinical escalation pathways with providers and hospital level leadership.
Qualifications: Education
  • Bachelor’s Degree in nursing or related field Required
Work Experience
  • 7 years of healthcare clinical leadership experience in a large and complex setting with direct responsibility for cost/revenue centers, designing and implementing business plans, and managing staff. Required
  • 5 years of experience leading complex strategic initiatives/processes, which impact multiple departments with demonstrated results Required
Licenses and Certifications
  • RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure if nursing degree, RN license Required
