REPORTING RELATIONSHIPS:
Reports to the Facility Administrator, Charge Nurse and nursing staff.
QUALIFICATIONS:
- High school diploma or equivalent.
- Current certification or eligibility as a Patient Care Technician by a national certification organization(CCHT, CHT or CCNT). Certification must be obtained within 18 months of onset of experience as a PCT.
Minimum of 6 months of experience in acute or chronic hemodialysis obtained within the past 12
months. - Current CPR (cardiopulmonary resuscitation) certification.
- Read, speak, write and understand English.
- Ability to do simple math; addition, subtraction, multiplication, division. Normal color perception.
ESSENTIAL PHYSICAL DEMANDS AND WORK ENVIRONMENT:
Requires walking, standing, stooping and bending. Must be able to do lifting of at least 20 lbs. Requires
close mental, visual and auditory attention to patient. Work environment is characteristic of a health
care facility with air temperature control and moderate noise levels. May be exposed to infectious and
contagious disease/materials.
JOB SUMMARY:
The Patient Care Technician will administer quality patient care in accordance with facility policies and
procedures while working collaboratively and communicating with members of the interdisciplinary
team. Delivers care to adult chronic hemodialysis patients under the direct supervision of the licensed
nursing staff. Assists with technical aspects of hemodialysis care such as water system monitoring,
bicarbonate mixing, and equipment calibration. Works to ensure the attainment of patient outcomes
that meet KDOQI goals while following regulatory requirements of the Texas Department of State Health
Services and the Centers for Medicare/Medicaid Services. Behaviors foster a team approach to patient
care delivery.
ACCOUNTABILITIES AND PERFORMANCE OBJECTIVES:
Customer service and Professionalism:
- Consistently provides customer service as it applies to all internal and external
customers, including patients and families, coworkers and physicians. - Promotes a positive morale through demonstration of consistent courtesies.
- Completes a minimum of 5 hours of renal-related education annually.
- Maintains a professional appearance and practices good grooming habits while
adhering to the facility dress code.
Consistently meets facility attendance standards.
- Demonstrates flexibility in response to unexpected changes to work volume,
acuity, staffing and scheduling. - Completes other duties as assigned.
Patient care, planning, implementation and evaluation:
- Communicates accurate and pertinent information to the nursing staff in a timely
manner.
- Collects patient and machine data, makes pertinent observations and documents
accurately on the treatment record and medical record. - Provides reinforcement of patient education that has been initiated by licensed
personnel. - Demonstrates competence in the initiation, monitoring and termination of
hemodialysis treatments. - Evaluates patient responsiveness to treatment and promptly advises nursing staff
of changes in patient condition as determined by established reporting
parameters. - Performs procedures and administers medications as delegated by the Medical Director and in accordance with facility policies and procedures.
Organization and Problem Solving:
- Consistently documents daily treatment and medication charges for billing.
- Organizes and prioritizes work to utilize resources wisely.
- Completes assignments in a timely manner minimizing overtime.
Leadership and Teamwork:
- Consistently demonstrates teamwork and sensitivity toward coworkers by assisting
as needed. - Participates in orientation and development of new personnel.
- Consistently demonstrates respect of individuals and cultural beliefs and lifestyles
of patients/family members and coworkers. - Participates in the Quality Assessment Performance Improvement (QAPI) process
through conducting audits and data collection as requested and making recommendations on quality
improvement strategies. - Maintains open communication to assure that the Facility Administrator and Charge Nurse are advised of issues and concerns.
- Oversees completion of clinic specific tasks as assigned.
Safety:
- Consistently provides for patient safety needs and identifies risk factors and age-
appropriate safety measures. - Practices good body mechanics and correct methods of moving, lifting and
transporting patients. - Assures a clean, safe environment at all times.
- Consistently assures that equipment functions properly prior to and during patient
use. - Promptly reports any equipment problems or physical plant maintenance to the
appropriate personnel. - Adheres to all infection control policies.
- Consistently notifies personnel of potential hazards.
