NetSpend - Get paid early: Tap into 50% of your earnings before payday. * Ministry Program The External Scheduler is responsible for the timely scheduling of transportation for Senior CommUnity Care (SCC) participants to and from the Day Center and/or appointments. The Scheduler is also responsible for scheduling outside medical appointments for the Senior CommUnity Care participants and notification of appointments with participants, their family and home care providers. QUALIFICATIONS: * Education: High school diploma or equivalent required; Valid Driver's License in state of hire. * Experience: Have a minimum of one years' experience in working with the frail elderly. * Skills and Knowledge: * Able to read and understand maps and routing information to navigate. * Able to handle information in a confidential manner at all times. * Able to deal professionally and diplomatically with people. * Must have good interpersonal skills. * Must be flexible, and able to function in stressful situations. * Knowledge of safety requirements pertaining to the transportation industry. * Requires individual who can coordinate multiple tasks simultaneously. * Skilled in establishing and maintaining effective working relationships with participants, caregivers, drivers and co-workers. * Proficient in identifying and recommending problem resolution. * Ability to work independently with minimum supervision. * Common sense problem-solving. * Strong organization skills and strong attention to detail with the ability to prioritize. * Ability to successfully complete driver training program for an occasional backup driver when needed. * Ability to utilize computers and other electronic devices ESSENTIAL FUNCTIONS: * Familiar with Pace Care Scheduling Program to be able to gather information needed for timely and efficient individual transportation and home care schedules. * Considers the needs of the participant and requirements of the PACE Center. Communicates these changes to Transportation Manager. * Develops schedules to limit employee overtime hours as much as possible. * Schedules Transportation for participants to and from the Day Center, medical/specialist appointments, and activity outings, as requested by the Interdisciplinary Team via the plan of care. Also Schedules Transportation for last minute emergencies that arise. * Assists in scheduling vehicle maintenance and equipment to operate within manufactures recommendations. * Functions as a member of the Interdisciplinary Team when appropriate. * Communicates with Transportation Manager daily to be updated on participant conditions/ changes, plan of care changes, or Interdisciplinary Team requests. * Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines. * Maintains the confidentiality of all company procedures, results, and information about participants, clients or families. * Handles potentially infectious specimens wi h appropriate biohazard precautions, and practices Universal Precautions. * Participates in and supports Quality Improvement Initiatives. * Participates in continuing education classes and any required staff and training meetings. Maintains any required certifications or licenses. * Performs other duties as required or requested.
