The Facilitator ensures fidelity to the Wraparound model by coordinating and overseeing services for youth and families. The Facilitator promotes a strong family and youth voice in service planning and goal development, ensuring services are individualized, family-driven, flexible, community-based, and collaborative across systems.
Serving as the primary point of contact, the Facilitator coordinates care, leads the Wraparound process, and monitors implementation of the individualized Plan of Care. They maintain team cohesion, gather ongoing input from families and providers, and ensure quality service delivery, including crisis intervention and stabilization.
The Facilitator implements all four phases of Wraparound-Engagement, Plan Development, Implementation, and Transition-while upholding Wraparound principles and coordinating services across systems such as mental health, education, child welfare, juvenile justice, and tribal systems when applicable.
REQUIREMENTS:
- Must be able to pass California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.
- Must qualify as a Medi-Cal Specialty Mental Health Services (SMHS) provider. Eligible activities may be claimed under Medi-Cal if all state and federal Medicaid requirements are met.
- Completion of the Statewide Standardized Child and Family Team (CFT) Facilitation Training, accessible through Regional Training Academies, the UC Davis Resource Center for Family-Focused Practice (RCFFP), or can be completed after accepting an employment offer.
- Position requires driving traveling to site locations and outside locations as needed or requested, valid driver's license, a motor vehicle clearance, and proof of insurance is Must be maintained throughout employment.
EDUCATION:
- Bachelor's Degree in Social Work, Psychology or another human services field.
EXPERIENCE:
- Must have at least 2 years' experience providing mental health and social services with seriously emotionally disturbed youth and their families.

- Ability to respond quickly, effectively and calmly to client crisis and life situations.
- All direct service personnel will be expected to maintain a flexible schedule (which will not be traditional 9-5) and must be available to provide services in the community/in-home service as designated by the people who will be served within the High Fidelity Wraparound program.

- Basic computer skills and literacy are required to use the agency electronic health Staff must be able to log on, type, use a mouse and navigate through various screens/windows to complete daily chart documentation and chart supervision requirements.

DUTIES
- Meets weekly with supervisor and keeps supervisor informed of all aspects of the SCHARP High Fidelity Wraparound program in a timely manner.
- Is the agency point person for initiating first contact with caregivers on all newly assigned cases within 24 hrs. The first face to face meeting with the family will be scheduled at this time. The Facilitator should coordinate this first face to face meeting so they are accompanied by all HFW team members.
- Is responsible for completing documentation to open the case for DCFS & DMH services. This includes, but is not limited to, obtaining required consents, reviewing information to determine what client historical information is needed and obtaining Release for Patient Information.
- Utilizes awareness and working knowledge of the Integrated Practice - Child and Adolescent Needs and Strengths (IP-CANS) to conduct CANS with youth and families every 6 months, incorporating these ratings into planning and to provide ongoing updates.
- Leads development of Wraparound team agreements, team mission, the family vision statement, strengths identification, and development of the family's goals.
- Is responsible for organizing, mediating and documenting the monthly CFT meetings in the Plan of Care/Matrix. Update and review plans ensuring all team members are aligned and aware of responsibilities.
- Review and support meaningful family-focused discussion on any important documents and needed support at the time of the CFT, including regarding any flexible funds requests while documenting in the POC.
- Ensure that legal and ethical issues are addressed in the CFT. Track progress on action steps within each domain to ensure timely completion of responsibilities from one meeting to the next.
- Demonstrate the ability to work as part of a multidisciplinary team. This includes working with the assigned agency Wraparound team as well as staff from DMH, DCFS, Department of Probation, psychiatrists and other interagency and intra-agency staff to provide consistent services.
- Prioritizes unmet needs and translates them into measurable outcomes in the Matrix/Plan of Care (POC). The Facilitator ensures all the domains on the POC are discussed with youth and their families, establishing strategies and goals to meet unmet needs. The facilitator will ensure that the POC is reflective of family vision and provides emergency and/or additional services when needed.
- Completes required SCHARP check request forms for processing CRF (Case Rate Funds), forwards to supervisor for review and approval. Following the credit card protocol and process for funds.
- Develops client Safety and Crisis plans according to DCFS Works with HFW Team to coordinate and monitor DCFS Plan of Care and Safety Plans to ensure services are coordinated, proactive and reactive, non-duplicative, and are pertinent to the child and family's needs, desires, goals and objectives.
- Maintain contact with family members and significant other natural support persons of clients as Involve natural support persons at the request/approval of the youth and family receiving mental health services.
- Adhere to SCHARP documentation policy.
- Involve child/youth and their families in all aspects of the services they receive as partners, providing them with choices and with opportunities for empowerment whenever possible.
- Support transition planning from the onset, guiding the family through benchmarks toward sustainable support and success.
- Facilitator attends and participates in all pertinent SCHARP, DCFS, DMH and Department of Probation meetings for each client.
- Attend client children's court or probation court hearings.
- Other programmatic and administrative duties as assigned.
