FACT Therapist - Knox County

What You'll Do

• Provide individual, family, and group therapy using evidence-based, recovery-oriented practices.
• Complete behavioral health assessments and develop individualized treatment plans.
• Deliver community-based case management and connect clients to community resources.
• Respond to crises and advocate for client needs.
• Collaborate with psychiatrists, nurses, peer support specialists, and community partners.
• Maintain accurate clinical documentation and ensure compliance with agency and regulatory standards.
• Participate in the FACT team's rotating on-call schedule.

Qualifications

Required

• Bachelor's degree in Social Work, Counseling, or a related field
• Ohio LSW or LPC license
• Valid Ohio driver's license

Preferred

• LISW or LPCC
• Experience working with individuals with severe and persistent mental illness
• Community-based behavioral health experience
