FAMILY ADVOCATE PARTNER FOR RAPID SERVICE RESPONSE PROGRAM IN PHILADELPHIA

The Family Advocate Partner (FAP) plays a critical role in bridging the gap between families and the services we offer. With compassion, and practical support, the FAP helps families engage meaningfully with the program and navigate challenges with confidence.

The (FAP) provides hope, builds connections and linkages, to support the RSR Case Workers and at the intake home visit encourages parents to utilize their voice to be part of the family service plan. The FAP strengthens engagement, and supports skill-building consistent with RSR phases of service. Embodies Gemma’s UNITED values and the Gemma Way in all interactions with families and partners.

 The Family Advocate Partner (FAP) will:

  • Assist families at the initial home visit in joining with Gemma.
  • Support RSR Case Worker in navigating systems and accessing services (e.g., scheduling and accompanying appointments).
  • Services are focused on strengthening families by providing support services in the home and community throughout the intervention. This can ensure that skills develop throughout the intervention align with the skills needed for the families to succeed.
  • Support the Intake Specialist with connecting to families and follow-up, especially during high referral periods. (ie. Strive to attend all initial home visits.)
  • Connect with community supports; building connections across the city to support the needs of families. Share these connections and resources regularly with the RSR department at team meetings and via e-mail.
  • The Family Advocate Partner (FAP) will bring their expertise of navigating child-serving systems to join with parents and caregivers and support RSR Case Workers.
  • Participate in the Family Check Up Online training to understand the evidence-based practice framework and basis of the skill development component of the RSR program.
  • Completes documentation of services and outreach in a timely and accurate manner.

This is a hybrid role: Monday – Friday, full-time position that is based out of our Preheim Center in Southwest Philadelphia. Primarily in field. Flexible schedule is strongly preferred.

Experience and Education: Bilingual strongly preferred. High school diploma required, bachelor’s degree preferred. This position requires lived experience navigating the child welfare. 1-2 years’ experience preferred navigating the child welfare. Must be willing to utilize personal experience within the role.
