In this Family Case Manager role, you won’t just connect people to services, you’ll walk alongside families as they develop life skills and move toward long-term independence. You’ll work directly with families who have experienced homelessness and significant life challenges.
Compensation
Starting at $21.15/hour (based on experience and contract guidelines)
What You’ll Do (Your Impact)
- Serve as the primary support and advocate for families
- Conduct intakes and holistic assessments with empathy, professionalism, and cultural sensitivity
- Partner with families to co-create realistic, strengths-based service plans and actively support progress toward their goals
- Provide hands-on coaching, problem-solving, and supportive counseling to help families navigate real-life barriers and challenges
- Respond to crises with calm, sound judgment, and a trauma-informed approach
- Connect families to community resources and services and follow through to ensure access and engagement
- Maintain accurate, timely documentation that reflects both compliance standards and the story of each family’s progress

Qualifications
- High School Diploma or GED required
- Minimum 2 years of direct case management or related experience working with disenfranchised individuals and families (including at-risk children)
- Knowledge of issues impacting formerly homeless or at-risk families, including domestic violence, substance use, mental health, and trauma
- CPI certification (or ability to obtain), CPR and First Aid certification
- Valid Driver’s License with a clean driving record
Schedule
Full-time, generally Monday–Friday during business hours
(Some evenings, weekends, and holidays required based on program needs)
