Emplify Health by Bellin is seeking a Family Medicine Physician to join our established clinic in Pulaski. This is an excellent opportunity to practice in a team-based, patient-centered environment that supports both quality care and worklife balance.

• Full-time (1.0 FTE)
• Schedule:
  • 40 hours/week
  • 32 patient-facing hours
  • Typically 4-day work week
• Flexible clinic scheduling
• Manageable patient panel with opportunity for growth

• Outpatient-only Family Medicine
• Care for patients of all ages (newborn through geriatrics)
• Team-Based Care Model to support efficiency and provider satisfaction
• Collaborative environment with experienced clinical and support staff

• Lab

• Access to imaging and specialty support within the Bellin network

• EMR: EPIC

• CME:
  • $5,000 annual stipend
  • 80 hours CME time
