Long-Term Care Physician

Location: Northeast, Wisconsin and U.P. of Michigan

Bellin Health is seeking a Physician to join our innovative My Home My Care program, serving patients across Escanaba, Iron Mountain, Peshtigo, Green Bay, and surrounding communities. This unique role offers the opportunity to provide high-quality, relationship-based care directly in nursing homes, assisted living facilities, and select home settings.

This is an ideal opportunity for a physician who values autonomy, meaningful patient connections, and a proactive approach to care delivery outside of the traditional clinic setting.

• Full-time (1.0 FTE), Open to part-time
• • 80% Nursing Home
  • 20% Assisted Living/home
• Mobile, community-based care model
• Provide on-site care in partner facilities and select patient homes
• Strong interdisciplinary team support

• Deliver transitional, acute, and routine care in:

  • Nursing homes
  • Assisted living facilities
  • Select private residences (case-by-case)

• Practice operates as a mobile clinic model, bringing care directly to patients

• Focus on managing chronic conditions and complex patient needs

• Collaborate with a multidisciplinary care team to provide proactive, coordinated care

This model enhances patient outcomes by eliminating barriers to access and ensuring continuity of care in familiar environments.

• 2-year compensation guarantee

• Paid Time Off:
  • 200 hours annually
  • 264 hours annually after guarantee
• CME:
  • $5,000 annual stipend
  • 80 hours of CME time

• Call schedule: 1:12 rotation (2-week blocks)
• No weekends or holidays
