As aFamily Medicine Nurse Practitioner or Physician Assistant with Healthy Living Primary Care, you will play a crucial role in delivering comprehensive, patient-centered primary care. You will join an integrated healthcare team dedicated topreventative medicine,holistic patient care, and leveragingadvanced technologyto enhance both physical and mental well-being for our patients in Folsom or Fair Oaks. Every day you will provideprimary and preventative care to a diverse patient panel, including adults and pediatric patients. Your responsibilities will include managing stable chronic medical conditions, conducting Medicare annual wellness exams, and performing various procedures. You will also conduct lab and imaging follow-up appointments, with ample time allocated for each patient encounter (30 minutes), ensuring quality of care over volume. To be successful in this role, you will be a board-certifiedNurse Practitioner (NP)orPhysician Assistant (PA)with a passion forfamily medicineand a holistic approach to patient care. You should possess strong clinical skills in managing common primary care conditions and be adept at leveraging technology to enhance patient outcomes.
