Job Summary and Responsibilities

As aFamily Medicine Nurse Practitioner or Physician Assistant with Healthy Living Primary Care, you will play a crucial role in delivering comprehensive, patient-centered primary care. You will join an integrated healthcare team dedicated topreventative medicine,holistic patient care, and leveragingadvanced technologyto enhance both physical and mental well-being for our patients in Folsom or Fair Oaks. This is an exciting opportunity for anAPRNorPA-Cto make a significant impact within a highly-ratedprimary care networkknown for exceptional patient satisfaction and physician autonomy, located in theGreater Sacramento Area.

Every day you will provideprimary and preventative care to a diverse patient panel, including adults and pediatric patients. Your responsibilities will include managing stable chronic medical conditions, conducting Medicare annual wellness exams, and performing various procedures. You will also conduct lab and imaging follow-up appointments, with ample time allocated for each patient encounter (30 minutes), ensuring quality of care over volume. To be successful in this role, you will be a board-certifiedNurse Practitioner (NP)orPhysician Assistant (PA)with a passion forfamily medicineand a holistic approach to patient care. You should possess strong clinical skills in managing common primary care conditions and be adept at leveraging technology to enhance patient outcomes. Job Requirements

Required

  • Completion of accredited training program(s)
  • Active California NP or PA license (or ability to obtain)
  • Board certified
  • 2-5 years prior APP experience in family medicine
  • Exceptional communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Commitment to providing compassionate and patient-centered care
  • Willingness to learn and grow

Preferred

  • Ability to speak Spanish and/or Russian
  • Prior APP experience seeing pediatric patients
