What You'll Do

Provide comprehensive primary care to patients of all ages, including preventive services, chronic disease management, and acute care Conduct thorough patient assessments and develop individualized treatment plans Prescribe medications and coordinate care with other healthcare professionals Educate patients and families on health promotion, disease prevention, and lifestyle changes Document patient encounters and outcomes using EMR systems Participate in quality improvement initiatives and stay current with clinical best practices

Qualifications

Master's or Doctorate degree in Nursing Active Nurse Practitioner license in Pennsylvania Board certification as a Family Nurse Practitioner (FNP) Experience in community health, outpatient care, or underserved populations preferred Proficiency in EMR systems and strong documentation skills BLS certification required; additional certifications (e.g., ACLS, PALS) are a plus
