TheFamily Medicine Nurse Practitioner will provide comprehensive primary care services to patients across the lifespan. Working within a multidisciplinary care team, the Nurse Practitioner will provide high-quality, patient-centered care with a particular focus on improving access and health outcomes for underserved communities on Chicago’s South Side.
The Nurse Practitioner will evaluate, diagnose, and manage acute and chronic conditions, provide preventive and wellness care, and educate patients on disease prevention and health maintenance. This individual will collaborate closely with physicians, advanced practice providers, behavioral health professionals, and other members of the care team to support the organization’s whole-person approach to care.
Essential Duties and Responsibilities
Provide comprehensive primary care services to patients across the lifespan, including children, adolescents, adults, and older adults.
Conduct comprehensive histories and physical examinations and assess patients presenting with acute and chronic health concerns.
Diagnose and manage common acute illnesses, injuries, and chronic medical conditions within the scope of practice.
Develop individualized treatment plans based on clinical findings, evidence-based practices, and patient needs.
Order, perform, and interpret appropriate diagnostic tests, laboratory studies, and screenings.
Prescribe medications and other therapeutic interventions in accordance with Illinois scope-of-practice requirements and organizational policies.
Provide preventive care, routine health screenings, immunizations, and wellness services.
Manage chronic conditions such as diabetes, hypertension, asthma, obesity, and other conditions commonly encountered in primary care.
Educate patients and families regarding diagnoses, medications, treatment plans, disease prevention, nutrition, and healthy lifestyle choices.
Coordinate referrals to specialists and other healthcare or community-based services when appropriate.
Collaborate with physicians, behavioral health providers, nursing staff, and other members of the multidisciplinary care team to promote coordinated, whole-person care.
Identify social, behavioral, and environmental factors that may affect a patient’s health and connect patients with appropriate organizational and community resources.
Maintain timely, accurate, and complete documentation within the electronic health record.
Participate in quality improvement initiatives and support organizational efforts to improve patient outcomes, access to care, and continuity of care.
Maintain compliance with applicable federal and state regulations, clinical standards, and the organization’s policies and procedures.
Education and Experience
Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program required.
Current and unrestricted Illinois Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) licensure required or eligibility to obtain prior to employment.
Current national board certification as a Family Nurse Practitioner (FNP-C or FNP-BC) required.
Previous experience providing primary care in family medicine, community health, or a similar outpatient setting preferred.
Experience working within an FQHC, community health center, or with medically underserved populations strongly preferred.
Experience managing both acute and chronic medical conditions across the lifespan preferred.
Experience working as part of an integrated or multidisciplinary healthcare team preferred.
