Position Highlights:

• Full-time opportunity with Monday–Friday schedule
• Flexible, customizable rounding schedule in post-acute care settings
• Average 15–20 patients per day across skilled nursing, inpatient rehab, and long-term care
• No primary night call
• Collaborate with interdisciplinary teams and hospice/palliative partners

Compensation & Benefits:

• Productivity-based model (Fee-for-Service)
• Estimated income range: $296,000–$345,000 annually with no cap on monthly bonuses

Candidate Requirements:

• Board-certified in Family Medicine, Internal Medicine, or Emergency Medicine
• Minimum of 1-year primary care experience
• Active Arizona medical license or eligibility
• DEA registration or ability to obtain
