Job Details

Key Responsibilities:
Provide comprehensive primary care to patients across the lifespan.
Diagnose and treat acute and chronic medical conditions using evidence-based medicine.
Develop individualized treatment plans that incorporate preventive and wellness-focused care.
Manage chronic conditions and monitor patient progress, adjusting treatment plans as needed.
Prescribe and manage medications in accordance with patient needs and regulatory requirements.
Coordinate care with other healthcare professionals and make specialist referrals when appropriate.
Educate patients and families on disease prevention, health maintenance, and treatment plans.
Build strong, ongoing relationships with patients and their families through continuity of care.
Participate in population health initiatives designed to improve health outcomes within the community.
Maintain compliance with applicable medical laws, regulations, and clinical standards.
Collaborate with an interdisciplinary team to ensure coordinated, high-quality patient care.

Experience and Licensure:
MD or DO degree from an accredited medical school.
Board Certified or Board Eligible in Family Medicine.
2+ years of experience in family medicine or a related primary care setting.
Evidence-based medicine
Chronic disease management

Medication management and prescribing
Population health
Valid Pennsylvania medical license.
Valid DEA license.
