We are seeking a highly skilled and patient-centered Family Nurse Practitioner (FNP) to provide comprehensive primary and preventive healthcare services across the lifespan. The FNP serves as an advanced practice provider responsible for assessing, diagnosing, treating, and managing acute and chronic medical conditions for pediatric, adult, and geriatric populations.
This role combines clinical autonomy with collaborative healthcare delivery and focuses on preventive medicine, health education, chronic disease management, and continuity of care.
Key Responsibilities:
Primary Care & Clinical Management
Conduct comprehensive health assessments, physical exams, and patient histories
Diagnose and manage acute illnesses, chronic diseases, and common medical conditions
Order, interpret, and evaluate diagnostic tests, imaging, and laboratory studies
Develop individualized treatment plans based on evidence-based practice
Prescribe medications and therapies within scope of practice
Perform routine wellness visits, preventive screenings, and immunizations
Manage chronic conditions such as hypertension, diabetes, asthma, and cardiovascular disease
Provide women's health services, family planning, and preventive care
Offer patient education on disease prevention, nutrition, and lifestyle management
Care Coordination & Collaboration
Collaborate with physicians, specialists, nurses, and allied healthcare teams
Refer patients to specialists or higher levels of care when necessary
Participate in care planning and interdisciplinary case discussions
Ensure continuity of care through follow-up visits and treatment monitoring
Support patient advocacy and long-term wellness planning
Documentation & Compliance
Maintain accurate and timely documentation in Electronic Medical Records (EMR)
Ensure compliance with clinical standards, regulatory requirements, and practice guidelines
Participate in quality improvement initiatives and patient outcome tracking
Maintain licensure, certification, and continuing education requirements
Requirements:
Master's or Doctoral degree in Nursing (MSN, DNP) with Family Nurse Practitioner specialization
Active Registered Nurse (RN) license
Active Nurse Practitioner (NP) licensure and national FNP certification (AANP, ANCC, or equivalent)
DEA registration and prescriptive authority (where applicable)
Strong clinical diagnostic and treatment skills
Excellent patient communication and education abilities
Ability to work independently and collaboratively
Proficiency in EMR systems and healthcare documentation
Preferred (Nice-to-Have):
Experience in family medicine, urgent care, internal medicine, or outpatient primary care
Experience with pediatric, women's health, or chronic disease populations
Bilingual or multilingual communication skills
Telehealth or virtual care experience
Experience with value-based care models and preventive care initiatives
Specialty certifications in chronic disease management or family health
