The Family Nurse Practitioner works under the general direction of the Director of Primary Care to provide a wide range of primary care services across the lifespan which encompasses health promotion, injury and illness prevention, diagnosis, treatment and monitoring of commonly encountered health problems and injuries, monitoring of stable chronic health problems, and community development and advocacy. Duties: Assessment and Treatment of Patients • Assesses patient health by reviewing records, taking histories, performing exams, and documenting findings. • Assesses and treats acute and chronic conditions and injuries for all ages. • Performs wellness assessments for adults and children. • Initiates and performs screenings, diagnostic tests, and procedures appropriate to the patient''s condition with the RN and Medical Assistant. • Counsels patients and families on health concerns, test results, nutrition, birth control, development, aging, substance abuse, and related matters. • Identifies cases needing specialist care and consults specialists for patients. • Returns patient calls about follow-up questions, lab results, and refills. • Prescribes medications according to clinical standards. • Signs or faxes refill forms daily. • Prepares and sends specialty referrals. • Reviews and interprets lab results. • Promotes integrated care to meet patient needs. Patient and Staff Education • Educates patients and coordinates DIHFS patient education programs, materials, and resources. • Identifies staff education needs and plans, implements, and evaluates programs to meet them. Documentation and Tracking • Maintains accurate charts, records, and documents; reviews staff materials for completeness and accuracy. • Records chlamydia results per Colorado Department of Health guidelines. Health Clinic Management • Manages and prioritizes clinic tasks, delegating to ensure timely, appropriate patient care. • Consults with health and related care providers, including public health nurses, social workers, and mental health workers. • Establishes and monitors clinic nursing care standards with staff, managers, and providers. • Initiates, implements, and evaluates patient care policies and procedures. • Identifies problems and recommends solutions. • Studies American Indian health changes, unusual conditions, new treatments, and medications. • Demonstrates quality nursing care, problem-solving, resource use, collaboration, and professional standards. • Plans and supports clinic and interagency coordination for continuity of care. Actively participates in staff meetings, in-service, outreach, and educational programs. Other duties as assigned. Qualifications: KNOWLEDGE, SKILLS AND ABILITY Knowledge of state laws and regulations on nursing and nurse practitioner scope of practice; responsibilities, professional principles, procedures, and nurse practitioners'' techniques, as applicable within specific licensure. Knowledge of symptoms and normal course of acute and chronic illnesses, and their treatment. Knowledge of Family Planning methods. Knowledge of normal growth and development and common illnesses of infants and children. Knowledge of standard medical record-keeping and HIPAA requirements. Knowledge of procedures, techniques, and tests for conducting comprehensive physical examinations. Knowledge of techniques and interpretations of standard laboratory tests, x-rays, and basic diagnostic procedures. Knowledge of substance abuse symptoms, mental and emotional illness symptoms, and intervention techniques. Knowledge of medications commonly used for a wide variety of medical conditions and their effects, reactions, and contraindications, proper handling and administration of medications. Knowledge of symptoms, prevention, reporting requirements, and treatment of child and adult abuse and neglect. Knowledge of normal and abnormal family dynamics; differing cultural, religious, social, and economic factors affecting community medical care patients. Knowledge of causes, control, detection, prevention, and treatment of common communicable diseases, including sexually transmitted diseases. Knowledge of community resources for medical care, mental health care, and social services. Ability to effectively communicate with Native American patients from various social, cultural, economic, and educational backgrounds. Ability to effectively intervene in mental illness, substance abuse, and adult/child abuse and neglect. Ability to gain the cooperation of patients in treating communicable and environmentally based illnesses. Ability to accurately evaluate and diagnose medical conditions, including obtaining and assessing medical histories. Ability to work independently, apply sound medical judgment and department protocols in treatment, treatment planning, and referral to specialists. Ability to recognize and take immediate action in emergency and immediate need medical cases. Ability to communicate clearly and concisely orally and in writing. Ability to maintain strict patient confidentiality. Ability to use computers and computer systems to input, edit, and change data in electronic health record.
