PATHS is seeking a Family Nurse Practitioner or Physician Assistant to join our collaborative primary care team in beautiful South Boston, Virginia. Practice evidence-based medicine in a Patient-Centered Medical Home (PCMH) where you'll build meaningful relationships with patients of all ages while enjoying a manageable patient panel and strong physician support.

Schedule
- Monday-Friday
- Full-time (40 hours/week)
- Rotating shared call for evenings and weekends
Responsibilities
You'll provide comprehensive primary care for patients across the lifespan while working alongside an experienced clinical team.
Responsibilities include:
- Perform patient evaluations, exams, diagnoses and treatment plans
- Manage acute and chronic medical conditions
- Conduct preventive care and wellness visits
- Order and interpret diagnostic testing
- Prescribe medications and manage refills
- Coordinate specialty referrals
- Review test results and communicate with patients
- Maintain timely documentation within eClinicalWorks
- Educate patients on disease prevention and healthy lifestyles
- Participate in quality improvement initiatives
- Attend provider and staff meetings
Qualifications
Required
- Licensed Family Nurse Practitioner or Physician Assistant in Virginia
- Current DEA registration
- Eligible to bill Medicare, Medicaid and commercial insurance
Preferred
- Board Certified
- FQHC experience
- Experience with eClinicalWorks
