Family Nurse Practitioner (FNP) – Primary Care Required Qualifications
Master’s Degree from an accredited Family Nurse Practitioner program
Current National Board Certification as a Family Nurse Practitioner
Active Nurse Practitioner license in the state of Ohio
Active NP/APRN license in good standing with no current or prior disciplinary actions
Minimum 1 year of recent Nurse Practitioner experience in primary care, family medicine, or urgent care
Strong understanding of quality measures related to chronic disease management
Ability to practice autonomously while collaborating with a clinical care team
Strong clinical assessment, diagnosis, and treatment skills
Excellent verbal, written, and electronic communication skills
Strong organizational skills with the ability to prioritize and multitask
Self-motivated with strong problem-solving abilities and adaptability
Proficiency with electronic medical systems and healthcare technology
Willingness to obtain additional state licensure if required

Job Description
Provide primary and family evidenced-based care, including assessment, diagnosis, treatment, and management of health problems.
Perform administrative duties such as opening/closing the clinic and maintaining inventory.
Handle patient appointments and conduct routine examinations to ensure positive health outcomes.
Communicate with patients about healthy lifestyle choices and coordinate care with other healthcare professionals.
Encourage patient participation in their own healthcare through services like health screenings and chronic condition management.
Critical Information
Must work three out of every six weekends with possible holiday rotations.
Remote work capability in a clinical care team culture.
Fingerprinting is not required for this position.
Education/Licenses Needed
Master's Degree level Family Nurse Practitioner program with current National Board Certification.
State of employment license to practice in the Advanced Practice Nurse role.
