Family Nurse Practitioner: Health Department

Position Summary: The Nurse Practitioner is assigned to provide primary care services to a full spectrum of patients from infants to the elderly, within the scope of their training and abilities, including health screening, health promotion, illness/disability prevention, acute illness care, and care of the stable chronically ill. The Nurse Practitioner is responsible for developing a collaborative relationship with the supervising physician and is also capable of performing defined procedures for which they have been appropriately trained.

Essential Duties and Responsibilities

• Performs physical examinations and maintains records appropriately.
• Orders diagnostic testing as needed, lab work, screening exams, and other specialized testing as needed and is responsible for notification to the patient of test results and arranging appropriate follow-up testing or consultation when necessary.
• Develops patient history and assesses risk factors, including hypertension, diabetes, smoking, substance abuse, and alcohol abuse.
• Includes patient and family members in plan of care, provides educational counseling and advocacy about health-related issues.
• Provide counseling and care during pre-conception, pregnancy, childbirth, and the postpartum period.
• Provide contraceptive counseling and advice.
• Provide routine gynecological services, including reproductive health visits, regular gynecologic care, and preventive care.
• Maintain patient records for clinical use, medical records use, billing use, and physician review.
• Completes all charting and medical record documentation, including progress notes, problems list, medication lists, allergy documentation, dictations, and quality assurance documentation, promptly.
• Serves as a resource person for other clinical staff.
• Promotes and participates in community health education and outreach programs.
• Collaborates with physicians, nurses, physician assistants, and other providers in the group practice and community.
• Assists in ongoing record review and improvement studies.
• Practices universal precautions and adheres to OSHA and workplace safety guidelines.
• Accepts and assumes responsibility for special projects as they arise, and all other duties assigned.
• Observes patient confidentiality and obtains appropriate release of information when necessary. Adheres to HIPAA requirements.

Family Nurse Practitioner Requirements and Experience:

• Knowledge of psychological principles, child and adolescent development
• Extensive knowledge of the health and aging process
• Demonstrates practical interpersonal communication skills
• Brings an innovative approach to work, can "think outside the box."
• Takes a team approach in accomplishing treatment goals
• Possesses strong clinical, interpersonal, and communication skills
• Demonstrates sound judgment in anticipating problems or adverse outcomes; takes appropriate corrective action
• Takes initiative to engage patients and other health professionals. Participates in activities that support continued competence as a family nurse practitioner
• Demonstrates knowledge and skill in providing age-appropriate care to children, adolescents, adults, and families.
• Advanced Practice Registered Nurse license in Indiana is required.
• All other duties as assigned
• Medical training for family practice, general practice, or other primary care practice orientation, with understanding and interest in preventative and community health.
• Ability to work with a team and take direction.
• Strong written and interpersonal communication skills.
