Position Summary


The Family Nurse Practitioner (FNP) provides comprehensive, patient-centered primary care services to individuals and families across the lifespan. This role focuses on health promotion, disease prevention, diagnosis, and management of acute and chronic conditions. The FNP works collaboratively with physicians and interdisciplinary teams to deliver high-quality, evidence-based care.


Key Responsibilities


  • Conduct comprehensive health assessments, including medical histories, physical exams, and risk evaluations
  • Diagnose and manage acute and chronic illnesses across all age groups
  • Develop and implement individualized treatment plans
  • Prescribe medications and therapies in accordance with state laws and organizational policies
  • Order, perform, and interpret diagnostic tests and laboratory results
  • Provide preventive care, including immunizations, screenings, and routine check-ups
  • Educate patients and families on disease management, medications, and healthy lifestyle practices
  • Manage chronic conditions such as diabetes, hypertension, and asthma
  • Coordinate care with specialists and other healthcare providers as needed
  • Maintain accurate and timely documentation in the electronic health record (EHR)
  • Participate in quality improvement initiatives and adhere to clinical protocols and guidelines
  • Ensure compliance with all regulatory, safety, and organizational standards
Qualifications


  • Master’s or Doctorate degree in Nursing from an accredited program
  • Current licensure as a Registered Nurse (RN) and Nurse Practitioner (NP) in the state of practice
  • Certification as a Family Nurse Practitioner (FNP-BC or FNP-C)
  • Current DEA license (if applicable)
  • Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS) preferred
Experience


  • Minimum of 1–3 years of clinical experience in primary care, family practice, or related field preferred
  • Experience in outpatient or clinic setting strongly preferred
Knowledge, Skills, and Abilities


  • Strong clinical assessment and diagnostic skills across the lifespan
  • Knowledge of primary care principles and chronic disease management
  • Excellent communication and interpersonal skills
  • Ability to build rapport with patients and families
  • Strong organizational and time management skills
  • Ability to work independently and collaboratively in a team environment
  • Proficiency in EHR systems and basic computer applications
