Position Details:

Hartford HealthCare is seeking a Full-time Family Nurse Practitioner or Physician Assistant for a multi-provider, primary care practice seeing patients of all ages. The position is located in Prospect, CT.

• Role: Open to Family Nurse Practitioners (FNP) and Physician Assistants (PA)

• Location: Prospect, CT

• Setting: 100% Outpatient Primary Care - Family Medicine, birth and up, no OB

• Schedule: Full-time, 4 days per week

• Call schedule: No call required

Nurse Practitioner and Physician Assistant Scope:

• Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

• In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

• Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the providers practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

Qualifications

Requirements:

• Licensure: Unrestricted State of Connecticut Medical License (or eligible)
• Training: APRN (Family Nurse Practitioner) or PA

Preferred:

• Experience:Two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA) in Primary Care or a related field seeing both peds and adults
