Terros Health is hiring a full-time Family Nurse Practitioner for a clinic in the Phoenix area.
Full-time; 40 hours/week
Schedule: 4-10's
Oak: 44th St/ Oak = 2 days/week (20 hours)
South Mountain: 36th St/ Baseline= 2 days/week (20 hours)

Salary Range: MSN: $115K-$118K+ DOE; DNP: $129K (depending on years of experience)
Additional Language Differential Pay Available: Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili or ASL: $3/hour ($6240 annually)
The Primary Care Provider conducts physicals/check-ups, medical evaluations, diagnostics, treats illnesses, orders lab tests, provides general and preventative care, prescribes and monitors medications, coordinates with other providers, makes specialty care referrals, educates patients about preventative care, establishes therapeutic relationships with a variety of patients for the purpose of establishing and reaching appropriate treatment goals, within the context of an integrated setting.

Minimum Qualifications
1.
AZ Nurse Practitioner License
2.
Master's degree in nursing or DNP and Certification
3.
Possess a valid BLS
4.
Possess a valid DEA license
5.
Must have valid Arizona driver's license, be 21 years of age with minimum 3 years driving experience, and meet requirements of Terros Health's driving policy
6.
Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
7.
Must pass a background check, TB Test and drug screen.
8.
Proficient using computer-based systems including electronic health records (NextGen preferred), Microsoft Outlook, and other Microsoft Office products.
