A health services network in New York City is currently seeking a licensed Family Nurse Practitioner (FNP) for a promising opportunity with their Brooklyn team as a Primary Care Provider. In this role, the Primary Care Provider will be responsible for screening, diagnosing, and treating patients for providing such care in a sensitive and non-judgmental manner, and for making appropriate internal and external referrals.
Responsibilities:
- Provide comprehensive care to patients according to pre-arranged schedules
- Document care promptly and follow charting protocols
- Ensure appropriate follow-up for all aspects of ordered care (e.g., specialty referrals, laboratory results, radiology reports, etc.)
- Provide care to patients living with HIV
- Participate in didactics, precepting, and case conferencing in order to build competency in HIV care
- Assist in continuous quality improvement through ongoing problem/needs identification and problem-solving
- Maintain familiarity with clinical guidelines and current treatment protocols in areas, such as transgender health, GEN care, and STI care
- Perform other duties, as needed
Qualifications:
- 1+ year of experience in a Medical Practice and/or similar setting
- NYS Family Nurse Practitioner (FNP) license
- Board Eligible / Certified in Family Medicine, Internal Medicine, and/or Infectious Disease specialty
- DEA Controlled Substances certification / registration
- CPR certification
