    About the Role

    Impact Life is seeking a compassionate and experienced Family Nurse Practitioner (FNP) / Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing integrated healthcare team in Dover, Delaware.

    This unique position provides both medical and behavioral healthcare across Impact Life’s Dover Rural Health Hub and Kent County Social Prescribing & Integrated Outpatient Services. The role supports a low-barrier, whole-person model of care for individuals with substance use disorders, co-occurring behavioral health conditions, unmet primary care needs, and complex social needs.

    The Nurse Practitioner will provide primary and preventive healthcare, behavioral health assessment and treatment, medications for opioid use disorder (MOUD), medication management, and coordinated care in collaboration with an interdisciplinary team.

    The Nurse Practitioner will play an important role in expanding access to care for individuals who may experience significant barriers to traditional healthcare while working alongside a multidisciplinary team committed to improving health and recovery outcomes.

    Key Responsibilities

    • Conduct comprehensive medical and behavioral health assessments, including physical and psychiatric evaluations.
    • Diagnose and treat acute and chronic medical and behavioral health conditions within scope of practice.
    • Provide primary and preventive healthcare, including screenings, chronic disease management, and specialty referrals.
    • Assess, prescribe, and manage medications for opioid use disorder (MOUD).
    • Prescribe and manage psychotropic medications and medications used to treat chronic health conditions.
    • Develop individualized, person-centered treatment and care plans.
    • Provide health education related to preventive care, chronic disease management, behavioral health, substance use disorders, and recovery.
    • Incorporate Social Prescribing approaches by identifying social needs and connecting participants with community resources and non-clinical supports.
    • Coordinate care across medical, behavioral health, SUD treatment, recovery, and community-based systems.
    • Collaborate with nursing, care coordination, community health, medical assistant, peer recovery, behavioral health, and community partners.
    • Monitor treatment progress and health outcomes and adjust clinical plans as appropriate.
    • Maintain accurate and timely documentation in the Electronic Health Record (EHR).
    • Participate in interdisciplinary consultation, quality improvement, and program evaluation activities.

    Requirements

    Required Qualifications

    • Current, unrestricted Nurse Practitioner license in the State of Delaware.
    • Dual certification as a Family Nurse Practitioner (FNP) and Psychiatric Mental Health Nurse Practitioner (PMHNP) required.
    • Board certification through ANCC, AANP, or an equivalent credentialing body.
    • Active DEA registration and prescriptive authority.
    • Ability to prescribe and manage medications for opioid use disorder (MOUD) in accordance with applicable federal and state requirements.
    • Minimum of two (2) years of clinical experience in primary care, behavioral health, substance use disorder treatment, or integrated care.
    • Strong understanding of integrated care models, social determinants of health, health equity, and recovery-oriented practices.

    Preferred Experience

    • Experience providing care to individuals with substance use disorders and co-occurring behavioral health conditions.
    • Experience working with individuals with complex healthcare and social needs.
    • Experience in community-based, rural health, low-barrier, trauma-informed, or recovery-oriented healthcare settings.
    • Experience with integrated medical and behavioral healthcare.
    • Familiarity with Social Prescribing, community resource navigation, and interdisciplinary care coordination.
