The Minute Clinic TelehealthNurse Practitioner delivers patient care services through a remote technologyplatform.
You will work in collaborationwith a dedicated team of professionals as you independently provide holistic,evidenced based care inclusive of accurate assessment, diagnosis, treatment,management of health problems, health counseling, and disposition planning forour patients ranging in age 18 months and above.
Encounters are documentedutilizing an electronic health record (EHR).

Duties
Patient Centered Quality andSafety
Evaluate primary care, acute,chronically ill, and transitional care patients, in addition to providinghealthcare education and counseling, and disposition planning for our patientsranging in age 18 months and above
Provide patient counseling;inclusive of pregnancy prevention, STI Prevention/safer sex practices,contraceptive care counseling and medication management
Educate patients on healthmaintenance and respond to patient care inquiries
Document all patient carewithin an EHR according to MinuteClinic policies and procedures
Provide care and coordinationof our patients with internal and external colleagues, including the broaderpatient centered medical home, ensuring the highest standard of care isprovided for all patients and at all times

Experience

Minimum of two years ofmedicallyrelevant experience
Basic Life Support (BLS)certification
A minimum of highspeed/broadband internet connectivity with a download speed of at least 25download and 3 upload speed. Speed test not required on resume, but pleaseensure candidate is aware of this requirement.
Must hold an unrestrictedlicense and have the ability to obtain multi state/ compact privileges andlicensure in noncompact states as required by the business.
