Position Overview:

ONE Health Ohio is seeking a Physician to provides direct high-quality care in a primary care within a Federally Qualigied Health Center (FQHC). Physicians deliver comprehensive evaluations, diagnosis, and treatment to a diverse population of patients.

Key Responsibilities:

  • Provide comprehensive medical care, diagnosis, and treatment of acute and chronic illnesses and conditions.
  • Educate patients and families regarding diagnoses, treatment plans, preventive care, and safety, ensuring adherence to HIPAA and other regulatory guidelines.
  • Collaborate with a team including nurse practitioners, physician assistants and other healthcare providers.
  • Collaborate/supervise and mentor mid-level providers and students as needed.
  • Support collaborative, multidisciplinary care with clinicians, behavior health providers and other team members.
  • Provide patient education on health promotion, disease prevention, and management of conditions.
  • Provide ongoing follow-up, continuity care and chronic disease management.
  • Maintain accurate and timely documentation in the EHR record system.

Ideal Candidates Will Have:

  • Board Certified of Board Eligible
  • Graduated from an accredited school of medicine.
  • Licensed to practice medicine in Ohio with good standing
  • Current DEA certificate
  • Completion of accredited residency program.
  • Valid Ohio drivers license
  • Current BLS Certification

Physical Requirements:
