What You’ll Do:
  • Conduct annual risk assessments and chronic condition reviews for pediatric and adult patients
  • Perform telehealth consultations and in-home visits (approximately 12–14 telehealth, 6–7 in-home per day; mixed schedule possible)
  • Manage patient schedules, providing care in home health settings or through telehealth platforms
  • Escalate urgent findings to primary care providers
  • Complete ongoing education modules during downtime
  • Help ensure patient health management and support

What You Need:
  • Active Oregon Medical License
  • Experience caring for pediatric and adult populations
  • Ability to perform telehealth and in-home visits
  • Strong communication and assessment skills
  • Preferred: prior experience in Telehealth or Home Health care
