What You’ll Do:
- Conduct annual risk assessments and chronic condition reviews for pediatric and adult patients
- Perform telehealth consultations and in-home visits (approximately 12–14 telehealth, 6–7 in-home per day; mixed schedule possible)
- Manage patient schedules, providing care in home health settings or through telehealth platforms
- Escalate urgent findings to primary care providers
- Complete ongoing education modules during downtime
- Help ensure patient health management and support
What You Need:
- Active Oregon Medical License
- Experience caring for pediatric and adult populations
- Ability to perform telehealth and in-home visits
- Strong communication and assessment skills
- Preferred: prior experience in Telehealth or Home Health care
