The Family Practice Physician is responsible for providing compassionate and quality clinical care for patients for the diagnosis, treatment and prevention of disease. They will examine patients, obtain medical histories, prescribe medications and perform tests and procedures to diagnose and treat injuries and illnesses, as well as counsel patients on diet, hygiene, and preventive healthcare.
Responsibilities:
Manage and maintain comprehensive primary care services for patients, including preventive care, acute illness, and chronic disease management.
Prepare and review patient medical histories and obtain clinical information through patient and family interviews, chart review, and examination.
Examine patients, develop diagnostic approaches, and order appropriate laboratory, diagnostic, and screening studies.
Analyze clinical findings and test results to formulate diagnoses, identify health risks, and guide treatment decisions.
Develop, implement, and monitor individualized treatment plans in alignment with clinical guidelines and patient needs.
Prescribe medications and therapeutic interventions as appropriate and evaluate their effectiveness over time.
Provide supportive and definitive medical care for patients across the continuum of health and illness.
Participate in care procedures within scope of training and demonstrated competency.
Provide continuity of care through follow-up, care coordination, and management of acute and long-term conditions, including triage of patient communications as needed.
Determine when consultation with specialists or other clinicians is needed and support coordination of care across services.
Establish and maintain collaborative relationships with physicians, advanced practice providers, and multidisciplinary care team members.
Communicate regularly with the healthcare team to identify high-risk patients, address care gaps, and support risk reduction strategies.
Recommend and coordinate appropriate referrals and community resources to support patient and family needs.
Provide focused care management for high-risk patients to reduce avoidable hospital and emergency department utilization and improve outcomes.
Maintain accurate documentation and registries related to care management activities, interventions, and patient care plans as applicable.
Educate patients and families on diagnoses, treatment plans, medications, preventive care, and wellness strategies.
Provide medication counseling for new prescriptions in accordance with established standards.
Promote patient advocacy, health literacy, and culturally and linguistically appropriate communication.
Build and sustain long-term, trust-based relationships with patients and families as part of community-centered healthcare delivery.
Address the whole person in care delivery, including physical, mental, emotional, and social health needs.
Maintain accurate, timely, and complete clinical documentation in the electronic health record in accordance with organizational and regulatory standards.
Participate in quality improvement, peer review, chart review, guideline development, and related activities to support safe and effective care.
Comply with patient confidentiality, privacy, and professional practice requirements.
Supervise, advise, and support clinical staff, trainees, and students as appropriate within scope and role.
Participate in departmental meetings, care conferences, and organizational activities as required.
Maintain and enhance clinical knowledge and skills through continuing medical education and professional development.
Provide guidance regarding the appropriateness and quality of clinical supplies and equipment when relevant to patient care.
Experience:
4+ years of practicing medicine in a clinic, hospital, or private practice preferred.
Adept at diagnosing problems and creating treatment plans for patients.
Excels in communicating sensitive and adverse information.
Sound judgment and expert decision-making abilities.
Analytical thinker and complex problem solver.
Working knowledge of FH clinical specific EMR - Centricity or EPIC.
Technological competency skills are mandatory.
Flexibility with managing multiple projects.
Attention to detail and dedication to accuracy.
Ability to handle confidential information in a professional & discrete manner.
Ability to communicate with patients, family, physicians professionally.
Education:
·M.D. or D.O. from an accredited Medical School, Board Certified
·Completion of a Residency Program in specialty
·Current and valid Illinois license as a physician and surgeon
·Current and valid DEA (Federal)
·Controlled Substance Registration Certificate (State of Illinois)
