The Family Service Coordinator II coordinates program services provided by the early intervention team, supports and advocates for families and children, and ensures compliance by monitoring written documentation.
Responsibilities
- Makes the initial family contact and gathers information to identify family concerns, priorities, and resources.
- Educates families of their rights and responsibilities.
- Completes Concerns, Priorities, and Resources (CPR) pages.
- Schedules evaluations and assists the evaluation team in determining program eligibility.
- Ensures compliance with the Individualized Family Service Plan (IFSP).
- Coordinates educational, community, and social services resources for families based on the IFSP.
- Assists in identifying social service needs for families and refers them to the program director for help in accessing community resources when appropriate.
- Coordinates upcoming due dates for six-month reviews, annual assessments, and transition reports; and schedules and coordinates regular evaluations, IFSP meetings, transition meetings, and closure meetings.
- Ensures a seamless transition of children from DDI Vantage to other agencies.
- Assists in child-find activities.
- Attends all mandatory Comprehensive System of Personal Development (CSPD) training for early intervention credentialing.
- Participates in DDI Vantage staff in-service training, attends workshops and conferences as approved.
- Completes written documentation in a timely manner to ensure program compliance.
- Attends team meetings as necessary and all mandatory DDI Vantage meetings.
- Demonstrates professionalism in all aspects of the job.
- Safeguards the confidentiality of all clients, at all times.
- Fulfills other duties as assigned.

- Education/Experience: The Family Service Coordinator II must have a bachelor's degree from an accredited college or university in early childhood education, family studies, health services or a related field. Experience working with children and families; knowledge of child development and family dynamics; and knowledge of community resources.
- A state and federal background screen, TB test, and motor vehicle records check will be conducted on all new hires.
