1.Work cooperatively with family services coordinator and bilingual intake/recruitment specialist to recruit children and families 2.Work cooperatively with family services coordinator and bilingual intake/recruitment specialist to collect all necessary documentation required in order to enroll a child 3.Work cooperatively with family services coordinator and bilingual intake/recruitment assistant to maintain enrollment in centers 4.Work cooperatively with center coordinator and center team to maintain clear communication on all center related issues 5.Provide daily support to classrooms within assigned caseload and document observations and interactions as needed 6.Provide ongoing support in daily center operations as needed 7.Complete an initial home visit and Family Outcomes Assessment on each family assigned to caseload 8.Assist each family assigned to caseload in developing and attaining goals 9.Maintain accurate, professional and up-to-date documentation on each family assigned to caseload within the program''s software systems 10.Conduct a minimum of two home visits per family during the academic school year 11.Conduct follow-up home visits per family on an as needed basis 12.Work with the center coordinator to schedule case conferences on each family 13.Document case conferences on each family and upload in program's software system 14.Follow-up on attendance concerns via phone calls, correspondence and / or home visits when necessary 15.Complete appropriate sections and review all injury reports originating within your center 16.Conduct all necessary follow-up work relative to injury reports with child and family; share with family services coordinator, and upload in program's software system 17.Complete appropriate section and review all family child information sheets 18.Conduct all necessary follow up work relative to family child information sheets with child and family, and upload in the program''s software system. 19.Review and complete appropriate section of all behavior referrals 20.Take all necessary steps as defined in the attendance policy and consult with family services coordinator prior to removing a child from active enrollment 21.Work cooperatively with agencies involved with each family assigned to caseload 22.Distribute accurate and completed status memos on necessary families in a timely manner 23.Attend in-house Support Services meetings 24.Attend all component & center meetings 25.Provide center staff with Child Abuse and Neglect training annually 26.Participate as directed by center coordinator, or other supervisor, as part of a Crisis Team in "Fire Drill / Evacuation Plan" or as part of any other support position
