About the role


Her Serenity Fertility Care is a growing integrative fertility practice dedicated to helping women navigate their fertility journey with personalized, compassionate care. We are looking for an experienced, empathetic, and bilingual (English/Spanish) Fertility Care Case Manager to join our team and serve as a key point of contact for our patients from the moment they reach out through every step of their care.


What you'll do


Manage patient onboarding, intake coordination, and ongoing case follow-up


Serve as the primary point of contact for fertility patients — in both English and Spanish — answering questions and providing support throughout their care


Coordinate lab orders, referrals, and follow-up appointments with clinical and specialist teams


Maintain accurate patient records and documentation in our EMR system


Communicate care plans clearly to patients in their preferred language, ensuring they understand next steps and feel fully supported


Assist with scheduling, prior authorizations, and insurance-related inquiries as needed


Work closely with the clinical team to ensure seamless continuity of care


What we're looking for


Fluent in English and Spanish — required. You will be working directly with Spanish-speaking patients on a daily basis


Minimum 2 years of experience in GYN and/or fertility care coordination or case management — this is a must


Strong understanding of fertility diagnostics, treatments, and patient support needs


Excellent communication and interpersonal skills — you genuinely connect with patients


Experience with EMR systems and patient documentation


Highly organized, detail-oriented, and able to manage multiple patients simultaneously


Compassionate, patient-centered approach — our patients are going through something deeply personal
