Pay Range: $28.00-$30.00 Per HR
Description:
• Join our team as a Field Case Manager, providing community-based support in the Homeless Services Division.
• This full-time, onsite position in Philadelphia requires strong communication skills and the ability to work independently.
• The role involves substantial community interaction and requires familiarity with general technology tools.
Requirements:
• Provide community-based support to individuals, ensuring compliance with service contracts and procedures.
• Conduct home-based visitations and community outreach to engage participants.
• Collaborate with a multidisciplinary team to deliver individualized services.
• Assist participants in developing and monitoring personal goal plans.
• Maintain daily productivity standards.
• Possess a valid driver's license with an acceptable driving record.
Qualifications:
• Bachelor's degree in sociology, social work, psychology, or related field.
• Strong verbal and written communication skills.
• Experience establishing supportive relationships with individuals with mental illnesses.
• Ability to work both independently and as part of a team.
• Proficiency in general technology tools (e.g., email, word processing).
• Experience with electronic health records preferred.
