    Field Nurse 

    REPORTS TO: DON/ADON/RN Case Manager

    POSITION SUMMARY :

    Provides skilled services to restore and promote patient's health by completing the nursing process. Assures quality of care by measuring health outcomes against patient care goals. Maintains safe and clean working environment complying with procedures, rules, and regulations.

    QUALIFICATIONS :

    • Registered Nurse (RN) or Licensed Practical Nurse (LPN)
    • Must demonstrate excellent interpersonal skills; both written and verbal, time management, and Microsoft Office skills.
    • Clinical skills
    • Be detail oriented and organized

    ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

    • Assess and chart observations of the patient's condition at each visit.
    • Complete evaluation tasks, including reviewing medication and vital signs.
    • Administer physician-prescribed medication.
    • Provide education to patients and families on proper home healthcare procedures and strategies.
    • Coordinate with physicians and other individuals in the patient's plan of care.
    • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
