What You’ll Do

• Provide skilled physical therapy services in patients’ homes under the supervision of a Physical Therapist
• Implement individualized treatment plans to improve mobility, strength, balance, and function
• Assist patients with therapeutic exercises and functional mobility training
• Educate patients and caregivers on safe movement and home exercise programs
• Monitor and report patient progress
• Document visits timely and accurately
• Collaborate with the interdisciplinary team to ensure coordinated care

What You’ll Need

• Active Florida PTA License
• Degree from an accredited PTA program
• Current CPR Certification
• Valid driver’s license & reliable transportation
• Strong clinical and communication skills
Home health experience preferred
Medicare experience preferred
