Are you an RN who enjoys pediatric care but is looking for a more predictable schedule and meaningful one-on-one patient relationships?

CareGivers America - NEPA is hiring a full-time RN Case Manager to support pediatric patients and families throughout Luzerne, Wyoming, and Sullivan County. You'll serve as a trusted clinical partner to families, coordinate care, and help children receive high-quality support from the comfort of their home.

Your Role as a Pediatric RN Case Manager:

• Complete patient assessments, including OASIS data collection
• Develop and oversee pediatric care plans with physicians and families
• Supervise and support LPNs and caregivers
• Coordinate care with families and interdisciplinary teams
• Ensure clinical documentation is accurate, timely, and compliant
• Participate in quality improvement and staff education
• Promote safe, high-quality care in the home

Territories: Luzerne, Wyoming, and Sullivan County

What We're Looking For:

• Active Pennsylvania RN license (Required) - BSN preferred
• Active CPR Certification
• 1 year of professional nursing experience preferred
• Valid driver's license
• Pediatric or home care experience preferred
• Strong leadership and communication skills
• Comfortable working independently in the field
