Primary Objectives and Key Results (OKRs)



  • Objective 1 Ensure timely and accurate access to public benefits and community resources for assigned families.
  • KR 1.1: Complete eligibility screening and benefits intake for 95% of new referrals within 10 business days.
  • KR 1.2: Achieve successful application submission (complete & correctly filed) for 90% of benefits cases managed within 30 calendar days of intake.
  • Objective 2 Strengthen client engagement and continuity of supports.
  • KR 2.1: Maintain documented contact with 100% of active clients at least once monthly and according to the individualized service plan schedule.
  • KR 2.2: Reduce gaps in service (unplanned service interruptions >14 days) by 50% over baseline within 6 months.
  • Objective 3 Produce reliable, audit-ready case documentation and outcome tracking.
  • KR 3.1: Case records for 98% of active clients are complete and stored within the electronic record system within 3 business days of each contact.
  • KR 3.2: Submit monthly outcome reports with accurate metrics for assigned caseload on schedule 100% of months.
  • Objective 4 Support interdisciplinary service delivery and referrals.
  • KR 4.1: Make appropriate referrals to clinical staff, community agencies, or vendors within 5 business days for 95% of identified needs requiring external intervention.
  • KR 4.2: Achieve a 90% confirmation rate of appointment/referral follow-through (verified attendance or documented reason for non-attendance) within 30 days of referral.



Key Performance Indicators (KPIs)



  • Timeliness: % of intakes completed within target timeframe; average days from referral to first contact.
  • Accuracy and completeness of documentation: % of files meeting documentation standards on audit.
  • Benefits enrollment success rate: % of clients successfully enrolled in targeted benefits or programs.
  • Referral effectiveness: % of referrals resulting in initiated services and documented follow-up.
  • Client engagement and retention: % of active clients with required monthly contacts and service-plan adherence.
  • Service gap reduction: number and duration of unplanned service interruptions across caseload.
  • Reporting reliability: on-time delivery and accuracy of required monthly and quarterly reports.



Key Skills, Abilities, and Knowledge Required



Professional competencies that enable success in achieving the OKRs and KPIs include: strong knowledge of public benefits programs and eligibility rules; proficiency with client management and electronic health/case record systems; organized documentation and time-management skills; effective verbal and written communication with diverse populations; basic problem-solving and service navigation techniques; cultural competence and trauma-informed approaches; ability to collaborate with clinical staff and external agencies; and reliable remote-work practices including secure data handling. Relevant experience in case coordination, community resource navigation, social services, rehabilitation supports, or related fields is highly beneficial.
