Hours: 40 per week | Monday-Friday, 8:00 AM - 5:00 PM

Salary Range:

• LPN: $31.00 - $37.00/hr (Based on experience)
• RN: $36.00 - $44.00/hr (Based on experience)

This role is anything but routine. You'll float between our Somersworth and Portsmouth locations - supporting multiple primary care teams, learning new workflows, building cross-site relationships, and making a direct impact on patients from all backgrounds.

What You'll Do

As a Primary Care Nurse Float, you will:

• Rotate across all primary care teams, providing nursing support exactly where it's needed most
• Deliver compassionate, patient-centered care - in person and over the phone
• Support patient education, care coordination, follow-up calls, and result management
• Assist with clinical needs during nurse visits and provider visits
• Collaborate with MAs, providers, behavioral health, and family support teams
• Help ensure smooth patient flow and exceptional care experiences

Qualifications

• Current NH Nursing License (RN or LPN required)
• Experience in primary care or ambulatory care preferred
• Current BLS certification
• Strong communication, organization, and assessment skills
• Compassionate, team-oriented, and adaptable
