Responsibilities
- Assess wounds and foot conditions and develop individualized care plans.
- Perform wound debridement, dressing changes, and other appropriate wound-care procedures.
- Monitor wound healing, foot health, and patient progress.
- Support pain management related to wounds and promote patient comfort.
- Educate patients and facility staff on wound care, prevention, and foot-care practices.
- Communicate with facility staff, physicians, and other healthcare professionals to coordinate care.
- Maintain accurate and current clinical documentation.
- Travel to designated long-term care facilities in the assigned area.
Required qualifications
- Master’s or doctoral degree in nursing from an accredited program.
- Current Nurse Practitioner license valid for practice in Louisiana.
- Strong clinical assessment and treatment skills related to wounds and foot conditions.
- Ability to work independently, stay organized, and manage responsibilities across multiple facilities.
- Strong communication and interpersonal skills.
Preferred qualification
- Experience working in long term care settings
