Job Summary:

The PRN Physical Therapist provides high-quality rehabilitative care to residents in a skilled nursing facility on an as-needed basis. This role focuses on evaluating, diagnosing, and treating patients with mobility, functional, and physical impairments to improve independence, safety, and overall quality of life.


Key Responsibilities:

  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans

  • Provide skilled therapeutic interventions including exercise, manual therapy, gait training, and functional mobility training

  • Monitor patient progress and modify treatment plans as appropriate

  • Document all evaluations, treatments, progress notes, and discharge summaries in accordance with regulatory and facility guidelines

  • Collaborate with interdisciplinary team members including physicians, occupational therapists, speech therapists, and nursing staff

  • Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies

  • Ensure compliance with Medicare, Medicaid, and other payer guidelines

  • Maintain patient confidentiality and adhere to HIPAA regulations

  • Participate in care planning meetings as needed


Qualifications:

  • Doctor of Physical Therapy (DPT) or equivalent from an accredited program

  • Current state licensure as a Physical Therapist

  • Current CPR/BLS certification

  • Previous experience in a skilled nursing facility or geriatric setting preferred

  • Strong clinical reasoning and decision-making skills

  • Excellent communication and interpersonal skills

  • Ability to work independently and adapt to varying patient caseloads


Work Schedule:

  • PRN (as needed) based on facility census and staffing needs

  • Flexible scheduling; may include weekdays, weekends, and holidays


Physical Requirements:

  • Ability to lift, transfer, and assist patients with mobility (up to 50+ lbs with assistance as needed)

  • Prolonged standing, walking, bending, and reaching

  • Ability to respond quickly to patient needs and emergencies


Work Environment:

  • Skilled nursing facility setting

  • Exposure to patients with acute and chronic medical conditions

  • Fast-paced, team-oriented environment


Compensation:

  • Competitive hourly PRN rate

  • No guaranteed hours; schedule based on facility demand
