Overview:

VNS Health FT Behavioral Health RN Provides mental health assessment of and treatment to patients with psychiatric symptoms, cognitive deficits, and/or behavioral problems. Provides support and education to VNS Health staff and caregivers to enable them to better care for patients with psychiatric, cognitive and/or behavioral problems. Works under moderate supervision.

What You Will Do

 

  • Conducts psychosocial assessments and living conditions of patients and develops treatment recommendations.
  • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient. 
  •  Transports and utilizes required medical equipment and supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient homes/care facilities, VNS Health offices and other locations.
  • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals.
  • Provides nursing care in accordance with Agency policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care.
  • Provides any of the following services to patients: advocacy, psycho-education, cognitive/behavioral interventions, monitoring of mental status, emotional support, brief psychotherapy, family based interventions.
  • Provides crisis intervention as necessary.
  • Consults with Program Coordinator as necessary to develop psychiatric evaluation and treatment plan. 
  • Consults with referrer on assessment, recommendations, and update on progress of treatment.
  • Provides education, consultation and support to VNS Health staff. 
  • Assist the health care provider in implementing the plan of care.
  • Follows a treatment plan, revising and updating as necessary.
  • Makes appropriate referrals for mental health services.
  • Participates in interdisciplinary team meetings, case conferences and clinical supervision. 
  • Attends in-service education programs.
  • Assumes responsibility for continued professional growth. 
  • Maintains productivity sufficient to meet VNS Health Home Care standards. 
  • Maintains records in accordance with AMH and VNS Health record keeping standard and performs established workflow procedures. 
  • Participates in special projects and performs other duties as assigned.

 

About the Program

Assertive Community Treatment (ACT)

The Assertive Community Treatment (ACT) program delivers intensive, multidisciplinary behavioral health services to adults with serious mental illness who have a history of repeated psychiatric hospitalizations and difficulty engaging in traditional outpatient care.

Qualifications:

Licenses and Certifications:
License and current registration to practice as a Registered Nurse in NYS required As determined by operational/regional needs, a valid drivers license may be required National Provider Identifier (NPI) number required or must be obtained upon hire. Management will provide assistance with the NPI application process required 

Education:
Bachelor's Degree in Nursing required Master's Degree in Psychiatric Nursing preferred

Work Experience:
Minimum of one year of recent inpatient, outpatient or home care psychiatric experience required As determined by operational needs, bilingual skills may be required 

Pay Range per Visit:
If you are applying to the per diem per visit version of this job, the hiring range is as follows: $69.34- $110 per visit.

Pay Range: USD $118,942.00 - USD $152,717.00 /Yr.
