Rate range: $43 - 55/hr
Hilltop Healthcare Rehab & Skilled is seeking a dedicated and compassionate Occupational Therapist (OT) to join our exceptional in-house rehabilitation team in Duluth MN. Our therapists are committed to delivering person-centered care that helps residents improve mobility, regain independence, and achieve the highest possible level of function and quality of life.

Key Responsibilities
Evaluate residents’ functional abilities and their impact on daily activities
Develop and implement individualized occupational therapy treatment plans
Provide skilled therapy services focused on independence in activities of daily living and instrumental activities of daily living
Recommend adaptive equipment and home modifications as appropriate
Monitor resident progress and adjust OT treatment plans as needed
Educate residents, families, caregivers, and clinical staff
Collaborate with the interdisciplinary care team
Maintain accurate, timely, and compliant documentation
Ensure services meet all professional, legal, and regulatory standards
Qualifications
Active Occupational Therapy state license in good standing
Skilled nursing or long-term care experience preferred
Strong clinical, communication, and organizational skills
Knowledge of the Net Health Electronic Documentation System preferred
